Our goal is to make the evaluation process clear, supportive, and stress-free by ensuring families understand each step, from scheduling and testing to results, recommendations, and ongoing support. From selecting services to reviewing final results, we are here to guide you through every stage of the journey.

Academic Achievement and IQ testing for Gifted or Intellectual Disability evaluations are typically completed in one appointment, with verbal results provided the same day and written reports mailed afterward. Some evaluations may indicate the need for additional testing to further assess attention, behavior, mood, social functioning, or academic concerns. Gifted programs often require both IQ and Achievement testing results.
Comprehensive Evaluations include cognitive (IQ), academic, behavioral, and social-emotional assessments. These evaluations typically involve two testing sessions and one feedback meeting to review results and recommendations. Reports may include diagnoses, interpretations, educational recommendations, and Section 504 accommodation guidance when appropriate.
Please note: Additional evaluation time may be recommended if further assessment of mood or emotional concerns is needed.

We offer weekday, weekend, and some holiday appointments, with morning testing (9:00–9:30 AM) recommended for children under age 10 and afternoon appointments typically beginning at 1:00 PM. Testing is generally completed by 4:30 PM, and all evaluations include both verbal and written feedback.
To schedule the evaluation, please complete the Registration, Consent for Treatment, and HIPAA forms (both parents’ signatures required on consent forms) and email them to info@DoctorWandaEppes.com or fax them to (888) 366-4008. A $100 deposit is required to reserve your appointment and is partially refundable with at least seven days’ notice. Once scheduled, you will receive a payment confirmation from MXMERCHANT and a Welcome Letter from info@DoctorWandaEppes.com containing appointment details, provider contact information, and additional forms needed for a comprehensive evaluation.
Parents, teachers, and clients age 11+ may also be asked to complete online questionnaires to help provide a thorough understanding of the client’s academic, behavioral, emotional, and developmental needs. Please submit any prior evaluations, report cards, testing results, or relevant legal documents to support the evaluation process.

Please arrive 5–10 minutes early to allow time for check-in and help ensure a relaxed testing experience. Clients should have a good night’s sleep and eat a full meal before testing. We also recommend bringing healthy drinks and snacks for breaks.
Transportation by another adult must be arranged with our office in advance.
Parents of children under 16 should remain in the reception area during testing and be available during breaks. Clients ages 16–17 must arrive with a parent or guardian on the first testing day.
At the end of testing, you and your provider will schedule a feedback session, typically 2–3 weeks later, either in person or via Zoom. Following the session, you will have the opportunity to review the report for accuracy before receiving the final PDF copy for your records.

Following your feedback session, you are welcome to contact us with brief questions by email. More detailed concerns, consultations, or school meeting participation may require a follow-up appointment and additional fees.
If you would like us to communicate with schools, medical providers, or other third parties, please submit a signed Authorization for Release of Information form available on our website.
Comprehensive evaluations are often updated every 3–5 years, especially for college accommodations and standardized testing, which frequently require recent evaluations completed after age 16 using adult testing norms.
We securely store reports and registration records for a minimum of 7 years. Additional copies or PDF records are available upon request for a $25 fee.

Please call (407) 677-4001 or (407) 414-8751. We have found that matching your schedule to our providers’ availability, and office space, works best by phone. We do not schedule via email, nor do we have an online scheduler. Our office Manager, Lewis Jones, will be happy to help you.

Our office is a cash-pay practice. We accept cash, checks, money orders, and major credit cards, including Visa, Mastercard, Discover, and American Express. We also accept Health Savings Account (HSA) cards that function like a credit card.

Upon request, we can provide a superbill, which is a detailed receipt that includes the services provided, diagnosis codes, and billing codes used by insurance companies. You may submit this document directly to your insurance provider to request possible out-of-network reimbursement, depending on your plan. Please note that reimbursement is not guaranteed, and coverage varies by insurance carrier and individual policy. We encourage you to contact your insurance company in advance to ask about your out-of-network mental health or psychological evaluation benefits.

We find that when parents complete all paperwork thoroughly in advance, an additional history interview is often not necessary.
Our goal is to save you both time and the additional expense. However, if your child has a complex medical and/or educational history, we would appreciate you preparing a Word document summarizing that history and submitting it along with your other documents.
If you feel a more detailed history review is needed, we can provide this service for an additional fee upon request.

Students already stabilized on medication should stick to their routine and stay on them for the evaluation. If scheduled for a comprehensive evaluation, and your doctor concurs, you may bring them on medication on the first test date and not on medication on the second test date.

Our office is HIPAA-compliant and will not share private information unless you provide written authorization by completing the AUTHORIZATION OF RELEASE OF INFORMATION FORM.

Yes, this is required both ethically and legally. A biological parent or legal guardian must accompany the student at the beginning of the first testing appointment. On subsequent appointments, the student may be accompanied by a relative or trusted family friend. A responsible adult must remain in the waiting area to assist with bathroom breaks, provide support during breaks, and be available if needed during the evaluation.
If your child is 16 years of age or older, the parent may leave after the rapport-building portion of the appointment (typically 20 to 30 minutes). If you plan to leave, please provide the contact information for anyone who may be arriving later or picking up your child.

No, the report belongs to you, the parent, or the adult client. This office warehouses the report for a minimum of seven years, but we do not release it to anyone else unless you instruct us, in writing, to do so.

It is possible that a diagnosis may not be identified. However, you will still receive a comprehensive report that provides valuable insight into your child’s strengths, challenges, and unique learning or behavioral profile. This information can help parents, teachers, tutors, and other providers better understand how your child learns, address behavior concerns when applicable, and implement strategies to support success.
